One thing I've learned from having our own business in the past (construction) is to never hire friends or relatives. We made that mistake and lived to regret it. They take full advantage of your friendship or being a relative and it leaves you filled with anger because you really don't want to "hurt their feelings". Hubby and one of my brothers don't talk anymore because they've done business together (hubby is the one who got ripped off in the venture). First rule in business is don't work with close friends and relatives. it will bite you in the rea end everytime.

As for your friend, I think the idea of going to a temp angency is brilliant! She's not reliable anyhow so I don't think she'll mind. Doesn't sound like she really wants the job in the first place is she bails most times. As for the personal side of it, I too would wait until after the holidays and probably until the next time she promises something to your kid or yourself. that would be the opportunity to bring it up and say something like "is this a for sure thing now or are you going to disapoint my son again?" kind of thing. That would get that talk all started.

I too don't particularly like people who say they'll do something then turn around and not do it (without proper notice of course). I find that ignorant and disrespectful.
